Before You Start
If you are about to complete your online Disclosure and Barring Service (DBS) Application Form please ensure you are prepared to include:
• a full (complete and continuous) 5 year address history which includes any non UK addresses
• full name history – forenames, middle names and surnames – including the dates of any changes
• that you set aside sufficient time available to complete the form. The time taken to complete a form will vary depending upon your number of addresses over the last five years, name changes since birth etc
• the form can be saved at the end of each page and returned to at a later date
• you will not be able to return to the form once it has been submitted
